To Patients New to Our Practice:

We appreciate your selecting us to advise you about your periodontal health; we enjoy what we do and are grateful for the opportunity to serve you.  We will call you 1-2 days before the appointment to confirm the appointment.  If we are unable to reach you and you cannot keep the appointment, please contact us at least 48 hours before your appointment to reschedule.  When we receive cancellations, we will make every effort, if requested, to give you an earlier appointment.

It would be helpful if prior to meeting us, you spent a few quiet moments reviewing your medical and dental history as well as how it might have affected your present periodontal status.  In addition, you might consider what you desire for your dental health, what you expect from us, and what questions you need answered.

We would greatly appreciate your completing both sides before your visit and bringing it with you.  This will assist our giving you excellent attention during your time here.  There is a fee for the initial consultation; there may also be an additional fee for any necessary x-rays.  If you have dental insurance, we would appreciate your bringing two (2) signed insurance forms; however payment is expected at the time of the first visit.
